A practical tool for employers

Overview

Survivor benefits are payable from the OPSEU Pension Plan in the event a member dies while they are still employed, including members who are on a leave of absence or receiving Long-Term Income Protection (LTIP). Employers must therefore notify OPTrust when a member dies by following the procedures in this section.

Death procedures

Employers must complete the following procedures to terminate a member from the OPSEU Pension Plan when a member has died prior to retirement.

  1. The employer must complete the Death of Plan Member (OPTrust 1063) form.

  2. OPTrust will contact the member’s next of kin or estate representative directly to request any required documentation.
